During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment.
What to Bring
If you are a new patient with Periodontal Specialists of Vinings, please plan to arrive at least ten minutes prior to your scheduled appointment time. This allows us to get all of the necessary information we require, and we appreciate your courtesy in helping us stay on schedule.
- Completed New Patient Forms
- Insurance card
- Picture ID, such as Driver’s License
- All current medications, including vitamins and supplements, or a complete list
- Name and contact information of your previous dental service provider
